Matchup Request Form (Due Before January 1st)

Step One: Log in with your team account before filling in this form. This allows you to return to the form and edit it if that becomes necessary.

Please fill out the form below with your mutually agreed opponent matchups using their team name as you see it on the Teams Listing page on this site. You may contact a team by clicking on the team name and looking for their officers / staff. Be sure the matchup is mutually agreed upon before submitting this form. You may choose the game day but not the game time. Only fill in the spaces necessary. Not all game spaces must be completed. If you're only playing one game on Friday, for example, fill in the first field and leave the second one blank. If you're playing two games on Friday then fill in both spaces.

Friday games start at 10:00am, 12:00pm, 2:00pm & 4:00pm.
Saturday games start at 8:00am, 10:00am, 12:00pm, 2:00pm & 4:00pm.
Sunday games start at 8:00am, 10:00am, 12:00pm & 2:00pm.

Please make travel plans to be able to play at any game time assigned to your team.
